The LTO Driver’s License is a primary identification card that you can use for various transactions in the Philippines. Your address on your license does not matter if you only use it for driving. However, it becomes important when you open new bank accounts, especially if it is your only valid ID with an address. The driver’s license must match the address on your bank application, or the bank will not accept it. You will have to show other documents that prove your current address.
That is why it is important to update your LTO Driver’s License with your current address. In this article, we will show you how to request a revision of records to change your address on file. We will also cover the documents and fees required for this transaction in LTO.
Benefits of Changing Address on a Driver’s License
There is no issue if your current address differs from the one on your driver’s license. The Land Transportation Office (LTO) will still honor it, allowing you to drive legally in the Philippines. This situation is quite common. For instance, when I obtained my driver’s license, I was single and residing with my parents. Consequently, my parents’ address is what appears on my LTO records. Even after getting married, moving out, and not updating my address for 11 years—despite two renewals within that period—I encountered no problems.
However, a complication arose when I applied for a bank account at one of the major banks in the Philippines. I couldn’t use my parents’ address on the bank application because we already have our own place. Due to the discrepancy in addresses, I had to present a barangay certificate along with my driver’s license.
This predicament could have been avoided if I had applied for a revision of records and changed my address earlier. Therefore, if you intend to use your driver’s license as proof of address or residence for various transactions, it would be more beneficial to update your driver’s license to reflect your current residence.
Requirements for Revision of Records-Change of Address
Before applying for a revision of records—change of address, have to prepare the following documents:
- LTO Driver’s License: Bring the original and a photocopy.
- Duly-accomplished Application for Driver’s License: Get this from your preferred LTO branch.
- Proof of Residence: To prove your current residence, you can present a copy of your internet, electric, or water bill, bank statements, or credit card bill statements. These documents must bear your name and address. If you don’t have any of these documents, you can present a barangay certificate from your barangay that has jurisdiction over your current address. Scan the document(s), then save it as a PDF if you intend to apply online.
How to Apply for a Change of Address Using LTO Portal
The best time to apply for a change of address is when your driver’s license is due for renewal. You can hit two birds with one stone by doing this. But if you really need to change the address on your license, you can apply for it anytime. Luckily, you can now do the application online. However, you still need to go to LTO to get your updated license and submit the documents.
STEP 1: You need to have an LTMS account. To create an LTMS account, please follow this guide: How to Create an LTMS Portal Account
STEP 2: Log in to the system with your 15-digit username and the nominated password. Click Licensing option and accept the LTMS Terms of Use.
STEP 3: Click the Driver’s License option, then tap Next. When the page loads, click the Revision of Records tab, then click Next again.
STEP 4: The portal will now ask you what record(s) you want to revise. Click the Address option, then click Next.
STEP 5: Provide the information about your current address. Type in the house number (if there is any), and the street/village/subdivision. Use the drop down buttons to select your province, city or town, and barangay. Do not worry if you do not know the ZIP code. The system will automatically fill it out for you.
STEP 6: Upload the needed proof of address. You can use either a proof of current billing, or a barangay certification. Please note that the document will be checked by an LTO officer. Make sure the file doesn’t exceed the allowed maximum file size of 2046 kb.
STEP 7: The next page will show the application form showing your client ID, type of application, and the current DL codes of your license. It will also shows the fees you need to pay for the request. Click Apply to continue.
STEP 8: Choose your preferred payment channel. Please note that the convenience fees differs from each mode of payment. So, your total fees will depend on your chosen payment channel. Click Proceed to Summary to continue.
STEP 9: After payment, the application will be submitted for verification and approval of LTO. Once approved, LTO will contact you for the next instructions on when to go to an LTO branch to get your updated license.
Change of Address in LTO Branch
If you prefer to apply in person, it is advisable to do so on Tuesdays, Wednesdays, or Thursdays as these days have the least number of applicants. Before visiting an LTO branch, ensure that you have all the required documents with you. Please note that only LTO District Offices and Licensing Centers cater to these types of applications. Driver’s License Renewal Offices (DLRO) located in malls cannot accommodate Revision of Records transactions.
STEP 1: Go to the nearest LTO District Office or licensing center that caters revision of records transactions.
STEP 2: Approach the guard on duty or the customer service desk and tell them that you want to change your address on record. They will likely give you the LTO Form No. 21, or the Application for Driver’s License form, along with a list of requirements and a queue number. Fill out the application form. Indicate the type of application you are doing by ticking the Change of Address box under Revision of Records.
STEP 3: Submit the completed form to Window 1 along with the proof or residence. The evaluator will check the documents are complete and authentic. After evaluation, he or she will submit it to the next window for further processing.
STEP 4: Wait for your name or queue number to be called by the cashier. Pay the required amount then go back to your seat.
STEP 5: When it is your turn, you will be called by the Window 2 for photo, signature and biometric capture. Go back to your seat again then wait for the Releasing window to call your name or queue number.
STEP 6: When your name is called, receive your updated driver’s license. Do not forget to sign the release form before you leave.
This process will take you two to three hours, depending on the number of applicants that day. However, due to the rising numbers of license renewal centers in malls, the queue in LTO district offices is now much shorter.
Total Cost of Change of Address in LTO

A good opportunity to change your address and save money is during a driver’s license renewal. You only need to pay an extra PHP 100.00 for the revision of records, on top of the computer fee and license fee that you are already paying for the renewal. This way, you get two transactions done for only PHP 292.63. If you change your address separately, you will have to pay another computer fee and license fee.
